Hi Nurse Julie! About 25 years ago, my wife's grandmother, Elvira, was actively dieing. We were all in the room on her last day. She was basically blind, ok? I was in a crouched position, 18' from her when she sat up, looked directly at me and said, "Bob, ask them for a chair, that can't be comfortable!" We all laughed. My mother in law, her daughter, Bev, was an RN. The Dr came in and Bev whispered something to him. He reached into his lab coat and produced a syringe. He gave Elvira a shot and about 5 minutes later she was gone. Did he give her an overdose? I asked Bev years later and she wouldn't answer. It's always bugged me so I thought I would ask you if this is SOP for the elderly? Love your channel❤!

Nurse Julie I agree with you on this in Ohio we don’t have this and I have stage 4 pancreatic cancer the medication companies frown on this highly as I work in pharmacy. These Medicines are medicines we give to save a persons life but we know the toxic doses do just what this type of end of life choice accomplishes. Pentobarbital and any of the barbitals usually accomplishes this. I am not sure about the benzodiazepines maybe midazolam. My grandma wanted this but we couldn’t offer it she died in hospice a few days later.

My uncle has it. He’s been on a plateau for a number of years. He’s at home, takes physical and speech therapy. A lot of talking is difficult, but he still goes out for short trips with his portable O2 tanks. He mows the lawn on the riding mower, goes out to church, goes out to eat at a nearby diner. My aunt has psoriatic arthritis and mobility issues, so they have a live-in LPN who assists them with cleaning, cooking, bathing, washing heavy bedding, etc. On days their nurse is off, their daughter who lives upstairs takes over. My uncle has already decided what he wants/does not want for end of life care, hospice in the home.

The biggest misunderstanding in our family came from the use of morphine. Many think the morphine is killing the patient, when in fact they are already dying, and the morphine is making it easier for them to breathe. This is what I learned from our local hospice counselor. My mother I believe suffered unnecessarily, because those caring for her withheld the extra dose of morphine, prescribed by the hospice nurse. The reason given was that she was unable to swallow, that she was basically unconscious but "struggling." I wasn't there but my understanding from our local hospice is that the morpine can be administered by mouth in a dropper, and it will absorb into the body. Just want to share this in case others here have a fear of using morphine. Please have this discussion with your hospice nurse.

My mom forgot how to eat things she had eaten for years - eventually refused to eat or drink. I was able to get my brothers in time to see her before she passed. Caring for someone with dementia can be exhausting - but satisfying. The most important thing for me to remember was to keep a sense of humor. We made a game out of her trying to remember our names and the names of our pets. We laughed about the diaper changes. After my mom passed, my husband was declining and passed from emphysema two years later. Physically, emotionally and financially devastating. But I got through it and I wouldn't change a thing.

I was a kid when my mother told me this story. She herself was a kid, around 1925, in Belgium. One of the local farmers was dying, so everyone in the community came around to say goodbye to him. He was able to speak with everyone, even the kids. When he passed, everyone came to the funeral. Even as a kid myself, I thought that was a wonderful thing for the family and friends and acquaintances to say goodbye face to face, person to person.

I had a aunt in Ohio, that I always referred to her as my 'favorite'. She had two sons that were doctors, the youngest being closest to me in age. We spent a lot of time with each other growing up. Because he was a MD he made sure her health and other daily needs were taken care of. My phone rand one night and the caller ID indicated it was her. She was close to 90 in age and my first thought was how did she get our number, with her having early dementia issues. We talked and her mind was surprisingly clear. She said she wanted to ask me something. What? Are you coming to my funeral ?? My mind stopped for a few seconds and I remembered her amazingly corny sense of humor. I asked her when are you planning to have this event?? We both laughed very hard at her question and my response. I told her I wouldn't miss it for the world. About 2 years later she did pass and I did attend. Fast forward in years and a lady from where we attend church was in hospice and my wife fixed a meal for the family. When we walked in the house her hospital bed was in the living room and she was so glad to see us. She called me over and told me she needed a hug from me. We hugged and she said I have a question. You guessed it. Are you coming to my funeral? I told her about my aunt's call asking the same thing and we both laughed very hard. And I told her the same thing. I wouldn't miss it for the world. She had a big smile when I said it and I didn't miss it. There is no right thing to say. Sometimes you have to wing it.

It's understandable, that people are afraid to die, particularly the atheists. I've been fortunate to have some OBE experiences and I know I can exist outside my mortal body. Plato's Cave is a good allegory, of experiences of more dimensions than what we normally experience, that of the three dimensions of space and the one dimension of time.

My sister, who had been recently diagnosed with cancer, went to a big fancy hospital in Michigan last month. I live in another state, and my other sister was with her, but I saw the scans and watched the labs. Her liver didn’t seem to be working. But she didn’t have jaundice, you get yellow if your liver doesn’t work, right? I know nothing about medicine, and this was a big, huge, fancy hospital with a world-renowned cancer center. They would tell us if she was going to die, right? I kept asking my healthy sister, who was with my sick sister, “what are they telling you? What’s the plan?” And my sister said, “Not telling us anything yet. Maybe they’ll tell us tomorrow.” This is a big, miracle working hospital with a huge cancer center. They were experimenting with my sister’s kind of cancer. They were working on it. Maybe they’d tell us tomorrow. Okay. That was Thursday. They didn’t tell us the plan on Friday, either. Then it was the weekend. Not a lot happens in hospitals on weekends. Maybe they’d tell us on Monday. On Monday, I called my healthy sister, and asked again, “What was the plan?” They hadn’t told her. Maybe they’d tell her on Tuesday. When they didn’t talk to us on Tuesday, I called a friend of mine, who is a doctor. I sent her the mychart labs. I sent her the pictures of the scans. She told me that, though she hadn’t seen my sister, the labs and the films looked very, very bad, and that she didn’t think this looked like something my sister could recover from. She said, “sometimes, when a family is in shock, they get passive. It would really help if you get a little assertive. If you can, when we hang up, call the unit secretary and ask for a palliative medicine consult, and then call me back and let me know that you did it.” And so I did. Palliative medicine saw my sister the next morning. They set up an online family meeting for Friday at which they told us my sister was close to death, and that we should come in. We got there at 10 PM Friday night. They put her on comfort measures Saturday morning. She died Sunday at 8:15 AM. So, my point here is, even actual doctors and nurses at big fancy hospitals with world famous cancer centers don’t know how to have end of life conversations. Oncologists Oncology nurses: if you are reading this, for the love of God, stop being afraid of your hospital’s palliative care service!!! If you are too scared or too busy to talk to a family, you have a whole, underutilized service in your hospital who specialize in doing just that. Make them your partners. Don’t make the family, who are coping with shock, denial, and fear, do your job for you. Bring Palliative Medicine on to begin with! If we had been told, my family could have spent more than a day with my sister at the end of her life-a day when she was so (appropriately) out of it with morphine that she couldn’t communicate with us. Her friends could have come to visit. Our goodbyes wouldn’t have been the rushed emergency that they became at the end. That big fancy hospital TOTALLY failed us-not because my sister died of cancer-everybody dies of something, and cancer can be a formidable opponent. That big fancy hospital failed us because they neglected to tell us what was going on.
